Electronic Health Records (EHR) are successfully providing us with mountains of transactional data and changing the way patients and providers operationalize and experience healthcare delivery, and Population Health is transforming from the concept of collective analysis of health status proposed in 2003 to a technology vendor segment tasked with doing something with all that data.
